Fitness Diary side of the page:

The fitness diary page is a broad-based form for use with many, many choices of movement. This is great for the Diabetic because it allows you to count anything!

Some of the spaces are filled in for purposes of illustration. The columns on this form are; time, program, class name or body area, cardio, heart rate/pulse, weight training (weight/sets/reps), distance or time (walk, run, swim, bike, hike, misc), leg lifts, etc., calories burned or time and mood. Then total.

At the bottom of this form there is also a place for daily notes or conversations with doctor/dietitian/healthcare provider/health care team/coach/personal trainer.

The  more you move around, either sitting or standing, the better you will feel!
